Thresholds & Transitions initial entry into the adult male incarcerated population will be at the Plainfield Re-entry Education Facility (PREF). The philosophy behind this unique program emphasizes meeting the specific, individual needs of the residents which are identified during an intensive two-week admission and orientation process. Every man must make a formal application to be considered and accepted to become a resident of this transitional phase of their incarceration (eligibility is restricted to men who have 12-36 months of incarceration remaining)
Issues related to family relationships, employability, substance abuse, anger control and relapse prevention are assessed and treatment/educational planning targets those specific needs identified. The T&T sessions will specifically address HIV, Viral Hepatidities, STD's and TB with a focus on prevention, testing and treatment.
To help facilitate a successful transition from PREF to community living, an opportunity will be provided to bring in partners and/or family members to dialog about information learned and skills developed and to help create a plan of action following release. Residents and partners/family members will also have the option to participate in counseling and HIV/STD testing at the time of release.
Other Indiana facilities where T&T programming will be provided is the Department of Corrections Parole and Work Release Divisions and the Marion County Jail.
